Create Username
Create Password
Confirm Password
First Name
Last Name
Date of Birth
Zip
Security Code
If you have any issues creating
your account because of security
code please contact our office
Note: your security code can be provided
by contacting Jiva Med Spa
Click “Create Account”
A screen will appear
“JUST IN CASE YOU FORGET YOUR PASSWORD”
You will select Security Questions and Answers
Select “Continue”
You have SUCCESSFULLY created your account
You will be navigated to the dashboard
Now that myPatientVisit™ Account is set-up
URL to Log-In after you have created your account: https://www.mypatientvisit.com/#/
You can interact with the office, your provider and complete forms!
You will need to username and password information to access the Portal
